Campus Room Request Form - Employees Only

Procedures

  1. Fill out the below request form. Note: If you are NOT a San Jac employee, please use this form instead.
  2. The college coordinator will notify you if the facility is available or not. If available, the coordinator will send you an email with the confirmation number, date, time, room number, etc.
  3. As needed, send an online form to request IT/AV setup. For police department services, email Dispatch.Central@sjcd.edu

*Please allow at least 48 hour notice for room requests. If 48 hour notice is not possible, requests are not guaranteed. Also note that rooms are not reserved until the room request form has been completed and you have received a confirmation number.
