Required Documents

Bring the following items with you to submit on the first day of orientation:
  1. NSO Checklist

    Please complete this form and bring it in on the first day of orientation.

  2. Texas Driver's License

    Copy of the front and back of your Texas Driver's License.

  3. Blue Card

    Upload a copy of the front and back of your Blue Card to CastleBranch, then bring the original Blue Card on the first day of New Student Orientation.

    If you do not yet have your Blue Card, bring your receipt of the Texas Board of Nursing outcomes letter indicating you may sit for the National Council of Nursing Licensure Examination for Registered Nurses (NCLEX-RN®).

  4. Passport Photo

    2x2 passport-size photo* with a plain background and your name printed or typed on the back of the picture.

    Central Campus:

    North Campus:
    *No black and white or paper copies.

  5. Medical Insurance Card

    Copy of the front and back of your current medical insurance card.

  6. CastleBranch Criminal Background Check

    Copy of your background check results from CastleBranch.

  7. CastleBranch Drug Test

    Copy of your negative* drug screen report from CastleBranch.

    *Result possibilities are dilute negative, negative, and positive. No diluted negative results accepted.

  8. Summary Report from CastleBranch

    Copy of your CastleBranch summary report showing your current status as "completed" with your name at the top.

  9. Physical Medical Record

    Use this form to upload your physical medical record to CastleBranch.

  10. Completion of Clinical Requirements

    Copy of your immunization record and completed physical exam form to complete your clinical requirements should be uploaded to CastleBranch.

  11. American Heart Association BLS (CPR) Certification

    Proof of your American Heart Association BLS (CPR) Certification should be uploaded to CastleBranch.

  12. Official Transcripts

    If you had a prerequisite course in progress when you submitted your application, you will need to provide an unofficial transcript showing completion of the coursework.

    • If you took courses at other institution(s), you must also submit an official transcript to Admissions and an updated GPS report to the Nursing department indicating your coursework has been transferred to San Jacinto College and has been accepted and evaluated by Enrollment Services.

  13. Nursing Student Clinical (Central Campus Only)

    In an effort to make your commute to clinicals easier, please complete this survey for your greater Houston area location preference. 

Immunizations

  • Measles, Mumps & Rubella (MMR) series and a positive titer*
  • Varicella (Chicken Pox) series and a positive titer*
  • Hepatitis B series and a positive titer*
  • A TWINRIX series and a positive titer can be used in place of the Hepatitis B requirement
  • Hepatitis C negative titer
  • Tdap (within the last five years)
  • TB skin test (expires each year and has to be renewed)
  • Influenza (flu shot, must be current)
  • Chest X-ray results (if applicable)

*Titer for MMR, Varicella, and Hepatitis B must be on file in your student record before the end of the first semester.

 

Orientation

To promote student success, you are required to attend an on-campus two-day nursing student orientation.  Please bring your own lunch and dress in business casual attire.

Day 1: July 22, 2025, 7:15 a.m. - 5 p.m. (registration begins at 7 a.m.)
The first orientation day will provide information about the application requirements, program overview, expectations, and resources.

Day 2: August 22, 2025, 8 a.m. - 4 p.m.
The second orientation day will focus on study tips, time management, test taking, and test anxiety.

Location: C.11-1081 Health Science Building

If you are unable to participate in orientation, please notify your assigned San Jac ADN staff member before the start of the orientation.

ADN Staff Assignment

Throughout your nursing education journey with San Jac, you will have your very own staff member available to assist you and ensure your success. Please note for Central Campus, you're assigned a specific contact based on your last name.

North Campus

TX Board of Nursing Approval (Blue Card)

Your name has been submitted to the Texas Board of Nursing (TBON). Be on the lookout for an email from IdentoGo with instructions on how to complete your fingerprint background check.

If you don’t see the email within two weeks of your acceptance date, please reach out to your assigned San Jac staff member. They will be happy to help you.

Once your fingerprint background check is done, TBON will send you an outcome letter, also known as the blue card. You’ll need to bring this blue card with you to nursing orientation, as it’s required for enrollment.

Uniforms and Lab Pack

You're required to dress in uniform and have a lab pack on the first day of class. We encourage you to make an appointment as soon as possible at one of these locations:
